I don't know what type of plant it is because it was in a pack of mixed freshwater bulbs, but the leaves sprout up into lily pads from the bulb Recently, the leaves turned red and now all the leaves are disintegrating from the outside!

This plant may not match your tanks lighting requirements. Many aquarium plants require high lighting, and it sounds like the plant you have does. Also, some of these plants require a CO2 injection in the water. Fish exhale CO2 and plants breathe it in, but many aquariums simply do not have enough dissolved CO2. You may also be lacking nutrients the plant needs. Have you tried an aquarium plant fertilizer? I doubt this will help, as the problem is most likely one of the first two I mentioned. Also, plants don't do well with aquarium salt, so if you have been adding that, that could be a problem too.

Sounds like the type of plant you have is an Aponogeton, though I can't say which type without knowing more. You may try to google Aponogeton and figure out which variety you have. This may give you a clue as to what the problem is. You can definately find out care, lighting, CO2, etc. If it's not a variety of Aponogeton, it's probably Nymphaea. Try to google this too.

plants are really sensetive to changes in ph or lighting. these are the two things that seem to kill mine the quickest. it is really hard to say without more details but most lillies need a lot of light. so it may just not be getting enough light.
