Practice does make perfect. Speaking enough with natives can help improve pronunciation and increase your vocabulary. Listening to conversational CD's or watching movies can also help. After you've played them once or twice try to repeat what is said soon after so it helps you remember ways to say certain things.
Using all skills is the best. Not only speaking and listening, but reading and writing. Keeping a journal and writing every day is a good way to increase vocabulary and strengthen grammar. Have a friend or teacher check it. Or make a pen pal that you can write to often who can check your writing for you. Reading short articles in or books can help as well.

Immerse yourself. Spend time with English speakers. Ask them a lot of questions. Hopefully they will be able to explain the idioms of the language. Read English books. Start with easy ones and then move up to the more difficult reading when you are ready. Watch English language movies and television. Oh, and language software.
Good luck!

keep at it, learn and listen every single day, when you come across a new word that you don't know write it down and write a few sentences with the new word you have learnt, eg,
"shoe" - something you wear on your feet.
"I bought new shoes today"
"I have black shoes"
"Her shoes are very nice"
This is what I do with german and japanese to help me remember new words...
hope that helps, good luck!! :D

Read books. Use a dictionary for all words you don't understand.
