How do I get proof of my measles vaccination?

In order to enter a graduate program in Connecticut, I need to provide proof of measles immunization, which I received in New York State in the late 1960s. I know the name of the hospital where I was born, but no one alive today knows the name or address of my pediatrician at the time.

Should I just get another shot, rather than trying to track down this information?

Probably by asking the health department or doctors office where you lived at the time received.
